While many people think of palliative care as end-of-life care, it is actually focused on helping patients understand and proactively manage their chronic or life threatening illness to live as well as possible for as long as possible.

This spring, heart problems landed Judy in Glens Falls Hospital where she was diagnosed with congestive heart failure and cardiomyopathy. It was then that she was introduced to the hospital-based palliative care team. Because of the palliative care she received at Glens Falls Hospital, Judy has been able to spend quality time in her own home with her family and loved ones.
