The Code Gray Debriefing Tool is used to evaluate a Code Gray response and initiate corrective/preventative measures to improve safety.
The tool was created by our Task Force to Reduce Physical Assaults, and is submitted by the Nurse Manager/department leader to our Weekly Physical Assault Review team.
