Your Patient-Centered Medical Home
As your Patient-Centered Medical Home, we aim to be your partner in care. We are committed to providing proactive, whole-person care, while helping you to take a more active role in your health and wellness. Your primary care doctor leads your care, with help from a care team dedicated to ensuring that you obtain all recommended health screenings on time, and that you have access to the specialists and information resources you need to prevent or manage chronic health conditions.

Many of our primary care offices co-locate with on-site specialists, behavioral health professionals and integrated care coordinators to help provide access to and continuity of a wide array of services. Any aspects of care that we cannot provide are arranged for you with carefully selected specialty providers who share the patient centered approach.
As a member of our Medical Home, you will be provided with care based on current medical evidence, easy-to-understand information, tools to help you manage your health, and timely access to your health information. For example, our new GFH MyChart online tool enables you to schedule appointments and communicate with your care team online, as well as to review your personal medical records.
To help us in optimizing the quality of the care you receive, we ask that you:
- Identify goals that are important to you and ask for assistance when you need additional help reaching them.
- Notify our practice of visits to the ER or hospital.
- Make our office aware of self-referrals to specialists or any care received outside of our practice. This ensures our ability to coordinate your care and enhance the safety and experience of care transitions.
- Provide an accurate medical history and make us aware of changes in your condition or medications
Resources we provide:
We're available to assist you in obtaining any non-medical community services you may need, and to answering questions related to your health insurance coverage. Medicare, Medicaid, P-CAP, Child Health Plus, Family Health Plus and most major insurances plans are accepted. We recognize that there may be times when patients in need of care will be unable to pay for the health care services provided and ask you to patients to please contact the Patient Financial Assistance Program at 926-5111 for assistance.
In addition, in partnership with the Adirondack Health Institute, Glens Falls Hospital assists patients and families to shop for, compare and enroll in health insurance coverage through the New York State of Health Marketplace. On-site services are offered in the following locations:

Glens Falls Hospitals' primary care offices have always been committed to providing you high quality care, close to home, from health care providers you know and trust regardless of your payment source or social circumstances. As your Patient-centered Medical Home, we take this commitment to an even higher level.
If you would like to join our practice we ask that your prior medical records be transferred to us in advance of your first visit. We invite you to call to speak to the Office Manager or any one our front office staff that are happy to assist you with the transition process.
If you are in need of clinical advice after your primary care office has closed, please call your local health center and you will be transferred to an operator who will contact a provider to return your call promptly. If you are having a medical emergency, please call 9-1-1.
