Your Patient-Centered Medical Home
As your patient-centered Medical Home, we aim to be your partner in care. We are committed to providing proactive, whole-person care, while helping you to take a more active role in your health and wellness. Your primary care doctor leads your care, with help from a care team dedicated to ensuring that you obtain all recommended health screenings on time, and that you have access to the specialists and information resources you need to prevent or manage chronic health conditions. We are also available to assist you in obtaining any non-medical community services you may need, and to answering questions related to your health insurance coverage.
As a member of our Medical Home, you will be provided with care based on current medical evidence, easy-to-understand information, tools to help you manage your health, and timely access to your health information. For example, our new GFH MyChart online tool enables you to schedule appointments and communicate with your care team online, as well as to review your personal medical records.
To help us in optimizing the quality of the care you receive, we ask that you:
- Identify goals that are important to you and ask for assistance when you need additional help reaching them.
- Notify our practice of visits to the ER or hospital.
- Make our office aware of self-referrals to specialists or any care received outside of our practice. This ensures our ability to coordinate your care and enhance the safety and experience of care transitions.
- Provide an accurate medical history and make us aware of changes in your condition or medications.
Glens Falls Hospitals' primary care offices have always been committed to providing you high quality care, close to home, from health care providers you know and trust. As your Patient-centered Medical Home, we take this commitment to an even higher level.
If you are in need of clinical advice after your primary care office has closed, please call your local health center and you will be transferred to an operator who will contact a provider to return your call promptly. If you are having a medical emergency, please call 9-1-1.
