In the spirit of our mission to improve the health of people in our region by providing access to exceptional, affordable, and patient-centered care, Glens Falls Hospital, an Affiliate of the Albany Med Health System, is committed to providing healthcare services to all patients based on medical necessity. For patients who require financial assistance or who experience temporary financial hardship, Glens Falls Hospital offers assistance and payment options, including financial aid, financial assistance and discounted care.  Short-term and long-term payment plans are offered.

Covered Service and Service Area 

Covered services include all Albany Med Health System services that are deemed medically necessary. These services are covered regardless of whether a patient resides within the Albany Med Health System’s geographic area. No application will be refused based on residency.

Excluded Service

Patient Financial Assistance will be utilized for all medically necessary services rendered by Albany Med Health System, except for elective procedures, cosmetic procedures, hearing aids, or dental and eyecare that normally are not covered by insurance. Excluded services are priced at packaged rates with no additional discount. All payments are expected at the time of service.  Charges from private Physicians/Providers (not employed by Albany Med Health System) who provide services are not covered under the program.   Our website lists all non-employed physicians.    FAP will not be applied to patient balances due to coordination of benefits issues, or the patient’s inability to provide the insurance with documentation they request or to provide the facility with the information needed to bill an insurance/carrier. Patients who opt out of available insurance coverage may impact their eligibility for Financial Assistance.

Short-Term and Long-Term Payment Plans

Patients who cannot pay some or all of their financial responsibility may qualify for short-term or long-term payment plans. The payment plans are interest-free and patient balances must be paid within a mutually agreeable timeframe.  Longer term payment plans are available if approved by a financial counselor for those patients who cannot pay their balances within ninety days.

Financial Assistance Policy

The Albany Med Health System offers financial assistance in accordance with this Financial Assistance Policy (FAP). This FAP was developed to comply with all federal and state rules and regulations, including IRS regulations 26 CFR §1.501(r) and NYS Public Health Law §2807-k(9-a).

Eligibility

A patient’s eligibility for Patient Financial Assistance will be based upon the size of the applicant’s family and is limited to those families whose income is less than or equal to 4 times [400%] of the current poverty level income guidelines and determined by the Community Services Administration.

Application Process

Patients seeking access to the Patient Financial Assistance Program will complete an application. During this process, the determination of eligibility will be made. Financial Counselors are available to work with patients in completing financial assistance applications to determine what assistance is available. This includes assessing eligibility for Medicaid and Health Insurance Exchange plans. For more information, click here to view the FAP brochure.

The Health Insurance Marketplace

The Affordable Care Act (ACA) requires everyone legally living in the U.S. to have health insurance beginning January 1, 2014. It also gives millions of individuals with too little or no insurance, access to health plans at different cost levels. The law also provides financial assistance to those who qualify based on family size and income. Please see a financial counselor at the facility where you receive care for more information.
