Risk Assessments
The Warning Signs of poor nutritional health are often overlooked. Use
the following checklists to find out if you or someone you know is at nutritional
risk.
Read the statements in each of the tables on the following pages. Add up the numbers in the "YES"
column for those that apply for you.
On the bottom of the pages are scoring sheets. Figure out where
your score falls and then read the advice that is given. If you have
questions or concerns about your score, please contact Glens
