Features
The Diabetes Self-Management program is an ADA-recognized comprehensive program
taught by Certified Diabetes Educators (CDE) with an emphasis on
providing the knowledge and skills necessary for people to manage their
diabetes.
The program includes ten hours of group classes and three
individualized visits with a CDE to assess your individual self-care
needs and to support your efforts in meeting your personal goals for
self-care and diabetes control.

Benefits
The Diabetes Control and Complication Trial has demonstrated that
optimal diabetes control is effective in delaying the onset and slowing
the progression of major diabetic complications.
Education in managing your diabetes is a proven benefit in achieving
control. This program is appropriate for those newly diagnosed with
diabetes or those in need of a “refresher” course who want to take
charge of their diabetes. Support persons are encouraged to attend.
Program Details
The program is presented by Registered Dietitians and
Registered Nurses who are Certified Diabetes Educators specifically
trained in diabetes and prepared to work with needs specific to the
person with diabetes. The program supports the care provided by your
Primary Care provider, it is not a substitute.
Pre and Post program appointments are scheduled individually.
The cost of the program is covered by Medicare and most major
insurance plans.
The program is offered monthly at various times and locations
