Our Mission
Our mission at the Regional Diabetes Center
is to provide high quality
diabetes and self-management skills training to improve the
quality of health and life of people living with diabetes in our region.
We believe education is a key component in effective control and
management of diabetes. People with poorly controlled
diabetes can regain control and minimize the risk
of future complications. People with long-standing
diabetes can
learn new techniques to maintain good glycemic control
and regain motivation to
follow their self-management plans.
Individual Consultation with Diabetes Center Staff
Our nurse educators and dietitians are available for one-on-one
sessions with patients who have specific knowledge and/or skills training
needs. These sessions are available in lieu of group programs.
Our outpatient programs are taught by Certified Diabetes Educators and
consist of several components which may be taken individually or in a
comprehensive group self-management program.
Diabetes Self-Management Education Program
The Diabetes Center offers a nine-hour group course in which patients
learn to become fully involved in their self-care.
Participants will gain knowledge and skills to assist in achieving optimal control
of their diabetes. The course
is appropriate for patients with long-standing diabetes as well as those
with recent onset. A support person is encouraged to attend at no
additional cost.
Nutrition Management
The Nutrition Center offers
individualized instruction based on current national standards. Dietary intervention for the
treatment of high blood sugar levels and other special needs such as
weight and lipid management is the initial step in treating these
problems. Early nutrition education may postpone
the need for medications and other
costly treatments.
Insulin Administration
Training
Getting started with insulin therapy requires
skills training and confidence building. Individual training is
available to those individuals new to this type of therapy.
Insulin Pump Training
Learn the skills needed to successfully use an
insulin pump. Individual education is provided by a certified pump
trainer. Carbohydrate counting, an important component in achieving good
control on the pump, is taught by a registered dietitian.
Pediatric Program
This is an intensive program for children with
diabetes and their parents. It provides the information and skills
necessary for the family to integrate diabetes self-management into their
lifestyle. Independence and confidence are gained as the
child and parents assume progressive responsibility for diabetes management with close daily
guidance from the staff.
Blood Glucose Monitoring
Blood glucose monitoring is a cornerstone of diabetes
self-management. Instruction in proper use of a meter, evaluation of meter
accuracy and patient technique as well as patterns of testing and
interpretation of data are provided.
Support Groups
Sharing, support, and problem-solving are important
components of monthly meetings. Groups are
available for adults as well as children with diabetes.
Diabetes Risk Assessments
Diabetes risk assessments are
available throughout our service area for
individuals who are concerned they may have diabetes or are at high
risk.
Campership Program
The Campership Fund gives children with diabetes
the opportunity to attend a camp just for them, discover more about
their condition and learn they're not different from other children.
Prevention Program
If you are at high risk for developing diabetes or
have pre-diabetes, we offer a program giving you the tools to delay or
prevent the onset of diabetes.
The Seven County Diabetes Network
The Regional Diabetes Center is a partner of
The Seven County Diabetes
Network, a state
grant-funded coalition that targets
individuals at risk for, or already diagnosed with, diabetes throughout
Warren, Washington, Saratoga, Essex, Hamilton, Franklin and Clinton Counties.
