Primary Care Centers
To provide primary care and act as a focal
point for a coordinated population-based approach to prevention and
early intervention and improving the overall health status of the local
residents

Glens Falls Hospital established
Whitehall Family Medicine and
Granville Family Health
in 1988 and 1996, respectively. In 1998, Glens Falls
Hospital added Broad Street
Medical Group. In 2000,
Fort Edward Internal Medicine
joined the Glens Falls
Hospital network of health centers,
providing adult medical care to central Washington County. In 2003,
Wilton Family Medicine was
established in northern Saratoga County. Also in 2003, Glens Falls
Hospital acquired four health centers in
Cambridge,
Greenwich,
Hoosick Falls and
Salem when the hospital that
previously operated these centers closed due to financial difficulties.
In 2004, Evergreen Health Center
in Corinth became part of Glens Falls Hospital. In
2005, Hudson Falls Internal
Medicine became the most recent site to join the Hospital's health
care network.
In addition to primary care, these centers offer local people access to laboratory,
radiology and urology services; substance abuse counseling; family and mental health
counseling, nutrition and diabetes counseling; and a broad array of prevention and
education programs, without having to travel to Glens Falls. On average, more than half of
the patient visits to these centers each year are made by Medicare, Medicaid or uninsured
patients.
