To obtain a copy of a birth certificate, please contact the City Clerk's office at City Hall, 42 Ridge Street, Glens Falls, NY 12801 or call (518) 761-3801.
 

To obtain medical records, please download a protected health information authorization form. Complete the form and mail it to Medical Data Services Correspondence Office, Glens Falls Hospital, 100 Park Street, Glens Falls, NY 12801 or fax it to (518) 926-5404. A statutory fee may be charged to cover the costs of copying, mailing and other supplies associated with your request.  For further questions, you can contact Medical Data Services at (518) 926-5400. Please keep in mind, however, that Glens Falls Hospital only keeps medical records on file for the past 25 years.
