Seven County Diabetes Network

Mission: The Seven County Diabetes Network is a coalition that is grant-funded through The New York State Department of Health. The Network targets individuals at risk for, or already diagnosed with, diabetes throughout Warren, Washington, Saratoga, Essex, Hamilton, Franklin and Clinton Counties. The Seven County Diabetes Network works collaboratively with healthcare facilities, public health departments and community organizations to increase public awareness of risks, prevention strategies, management issues and available local resources for people with diabetes or at risk of developing diabetes. 

Services provided at no charge to the community:

bulletprograms about diabetes prevention and management;
bulletreferrals to American Diabetes Association-approved Diabetes Self-Management Programs;
bulletaccess to a Certified Diabetes Educator through a toll-free telephone number for questions and information about local diabetes resources and programs.

Healthy Lifestyles (Free)

LOCATION: The Nutrition Center, 2 Broad Street Plaza, 2nd floor, Glens Falls
DATES:
Beginning Thursday, May 3; meets for four consecutive Thursdays (May 3, 10, 17, 24) and June 21
TIMES: 4:00 PM to 6:00 PM
TOPICS: The program is taught by a Registered Dietitian/Certified Diabetes Educator and will focus on prevention of Type 2 diabetes through healthy lifestyle choices, including eating and physical activity strategies.
