Seven County Diabetes Network

Mission: The Seven County Diabetes Network is a coalition that is grant-funded through The New York State Department of Health. The Network targets individuals at risk for, or already diagnosed with, diabetes throughout Warren, Washington, Saratoga, Essex, Hamilton, Franklin and Clinton Counties. The Seven County Diabetes Network works collaboratively with healthcare facilities, public health departments and community organizations to increase public awareness of risks, prevention strategies, management issues and available local resources for people with diabetes or at risk of developing diabetes. 

Services provided at no charge to the community:

Partners: Glens Falls Hospital* (lead agency), Adirondack Medical Center*, Alice Hyde Medical Center*, Elizabethtown Community Hospital, Public Health Departments in four counties, Cornell Cooperative Extension of Essex and Saratoga counties, Inter-Lakes Health, Saratoga Hospital, Hudson Headwaters Health Network, New York State Department of Health, Diabetes Prevention and Control Program.
