County Diabetes Network
Mission: The Seven County
Diabetes Network is a coalition that is grant-funded through The New
York State Department of Health. The Network targets individuals at risk
for, or already diagnosed with, diabetes throughout Warren, Washington,
Saratoga, Essex, Hamilton, Franklin and Clinton Counties. The Seven
County Diabetes Network works collaboratively with healthcare
facilities, public health departments and community organizations to
increase public awareness of risks, prevention strategies, management
issues and available local resources for people with diabetes or at risk
of developing diabetes.
